Here at the Union Spring Church we are a family that meets the needs of one another. Below, you will find some request forms to meet your various needs. After you fill out the form, you will receive notification that your request has been received, and a member of our ministry will contact you regarding your request.
Baby Dedication Form. Fill out a baby dedication form in order to meet with the pastor concerning dedicating your baby. This ceremony will be held quarterly during Sunday services.
Bereavement Notification Form. We need to know about your cares during this time. We desire to offer to you our support when you experience the loss of a loved one. If at all possible, spend a little time filling out this form so that we can support you and show you that we care.
Counseling Request Form. We understand that your cares sometimes seem too hard to bear. That is why we extend to you Christian counseling based upon the Word of God. Before you make a step to fix the situation, try bringing your situation to someone that can steer you in the right direction.
Facility Request Inquiry.Your event is important to us and we would like to assist you in this endeavor, but first we need to make sure that the date and space is available.
Accepted Facility Request. Please fill out this form only if your facility request has been accepted! You will also be able to pay your deposit and final facility fees here.
Sick and Shut-In Visitation Request. This family understands the importance of laying hands on the sick, because with the prayer of faith, the sick will recover. We would like to be there for you.
Special Event Request. We know that there are some events that happen in your life or your children's lives that you would like your church to be a part of. Just fill out this form and we will try to be there.
